Ready to make a change from your current mental health and psychiatric providers? Unsure how to proceed and what information you need to share? CMHRC can help smooth that transition with expert support and resources on how to make a clean transition between providers. 

 

 

What We Offer

 

CMHRC’s Transition Support is a one time fee for service program. Priced to be highly competitive, this service is designed to help patients and families bridge the gap between old and new providers. 

 

Whether you, or your child, is moving to a new area, aging out of a program, or just ready to make a switch, changing psychiatrists and therapists can be an intimidating task. You need to be prepared to communicate details of your treatment history quickly easily and without risking confusion. When you start with a new provider you want to be able to let them know what you’ve tried, what’s worked, what hasn’t, and what your hopes and expectations are. What you don’t want to have to do is start from scratch while you and the provider try to reinvent the wheel.   

 

CMHRC experts can ease you through this transition using insight and knowledge developed over decades of experience as mental healthcare practitioners. We’ll provide you with coaching and guidance on communication with provider,  equip you with a transition plan for your new psychiatrist or therapist, and advocate to them on your behalf. 

 

 

Included in the $395 fee:

  • A series of three confidential consultations:
    • First consultation to discuss your reasons for transitioning, expectations, and developing a plan for how to proceed; 
    • Second consultation to review treatment history, successes, failures, and goals;
    • Third consultation to review transition plan and receive coaching on how to communicate with your previous and new providers about the transition.
  • Development of a comprehensive transition plan document which outlines your child’s treatment history, successes, failures, and goals, which can be given to your provider to summarize your expectations for the continuation of treatment;
  • Compilation of relevant, peer-reviewed, published papers outlining diagnostic and treatment processes and why they are required (when applicable);
  • Education and coaching on how to present symptom descriptions to providers and prescribers;
  • Education and coaching on effective communication with providers and prescribers;
  • Education and coaching on strategies for advocating for your child with providers and prescribers;
  • One 30 minute consultation with your prescribing physician; and
  • One 30 minute consultation with your current therapist or counselor.

CMHRC is a 501(c)(3) nonprofit organization. The fees charged for our programs and services are kept at below market rates because we believe that good quality mental healthcare support should be available to as many people as possible. 

 

Charitable giving supplements the cost of running our programs and services and provides funding for our free resources, supports, and educational efforts.
